Inpatient Disease Management Program

I've recently been reading about "DMP"s and am wondering about efficacy & affordability among other things. i've reached a point where I am just burnt out fighting this disease, and the life of a professional patient has, well, become intolerable. Coordinating my care is 100% my job, and it's just become impossible.  I need a time out, with a coordinated, focused asessment of my situation and a realistic plan - one that can begin immediately. I'm hoping to find an inpatient situation that is somehow affordable, but I know that's about as realistic as good dental insurance.

After 44 years I think it's time to go a different way, and if I continue along this path I don't think my body is going to hold out a whole lot longer. TBH I'm not sure I want it to. The depression is as debilitating as the disease now, and mental health is about as affordable and available as cheap dental care. I may also be one of the lucky ones to lose health insurance entirely next year, or at best have the cost of it increase 800%. 

Anyone who has had any kind of experience with a place or treatment program similar to this, I'd be grateful to hear about your experience.
