Anal fissure and swelling

Hi, I have suffered meny fissures in my past due to my diet. I have not been diagnosed with crohns or anything and I know what the pain of a fissure feels like. This time it is different though, I would have the normal pain from poop but a few hours later I would have starp pain further up inside that would happen a few times then be ok and I had a small amount of brown/reddish discharge once but thats all. After a month of this it just swelled up and the pain was worse and all around. I tried to feel inside and it felt like there was a skin tag inside. I was told it was a hemorriod, then I went to a specialist but she didnt look inside as the fissure has reopened that day so she just told me it was all happening because of the fissure as it had re-opened twice. She told me the pain further up was from the fissure too but I have never had this pain with a fissure before and I had fissures for months that would not heal. This is the only place I see anyone talking about this and its really affecting my day to day as the pain is random and I cannot sit, stand or lay without pain when it is bad. I checked for the skin tag again but it couldnt feel it anymore. I have been given diltiazem that did help with pain I think and now I am trying nifedipine and it hasnt done anthing so far but its only been just over a week so it might take a while to help I dont know. Does anyone has any ideas about this? I have looked into this a lot and I am thinking it could be a fistula but I have no idea.
