9 year old son won't eat even taking periactin

My son has had Crohns for 4 years and has continues to fall on the growth chart. Started at the 90th% and is down to the 25th% in weight and now 45th% in height. We have tried everything.... protein shakes, meals every two hours, eat whatever you want, and he just doesn't eat. I noticed this week that he chews each bite of his food at least 100 times before swallowing. Which now explains why it can take him up to an hour and a half to finish a meal. So he eats slow and then gets full and therefore only consumes half of his food on his plate. He is even on an appetite stimulant (periactin) since October and it has not helped at all. So I'm confused.....
 Is it behavioral? medical? sensory issue? He says he has no favorite foods, and doesn't like to eat. he claims is never hungry even for ice cream! I will also add that he has not had a flare since he was diagnosed. Has been on iron supplements and pentasa for these 4 years. I'm not sure if that impacts his appetite in any way.
 Any suggestions?
